A novel percutaneous tracheostomy technique to reduce aerosolisation during the COVID-19 pandemic: A description and case series

COVID-19 is a global pandemic with many patients requiring prolonged mechanical ventilation. associated laryngeal oedema and high rate of reintubation difficult airway. Tracheostomy insertion an aerosol generating procedure, so we strived to make our novel technique safe for operator patient. To share experience percutaneous tracheostomy technique, based on case series 18 pneumonitis. Our landmark-based approach without bronchoscopic confirmation the correct needle placement. Blunt dissection using tracheal dilators onto rings facilitates first pass into trachea. The tube retracted supraglottic airway, cuff overinflated, wet throat pack inserted reduce aerosolisation. From March 2020 May 2020, 38 suspected or confirmed presented Royal Bolton Hospital invasive underwent tracheostomy. 6 have been decannulated, 12 died. Mean time from intubation was 6.1 days decannulation 20.6 days. No developed symptoms. Despite low numbers appears be safe, but requires larger controlled trial. As institution avoided difficulties reduced drug usage.
